Background:Crestor, a prescription drug, has been associated with increased mortality in patients with cardiovascular disease. A randomized controlled trial (RCT) was conducted to examine the association between Crestor use and increased mortality in the community. This study was a multicentre, observational, randomised controlled trial. The primary outcome was the risk of mortality, defined as the expected number of deaths or death from any cause within the next 30 days, over a period of six months. Data were obtained from 4,935 patients in the RCT. Of these, 3,716 (45%) died during the six-month study period. Data were analysed using the Cox proportional hazards model. Results: Of the 3,764 deaths, 2,852 (35%) were defined as death or non-adherence. The risk for death or non-adherence was higher in patients with a baseline heart rate of 55 or more/min (hazard ratio [HR], 1.14; 95% confidence interval [CI], 1.06 to 1.24;P=.001). In a multivariate analysis, the increased risk was greater in patients who were on statins and those who were started on statins (HR, 1.41; 95% CI, 1.25 to 1.51;=.001) and on low-dose aspirin (HR, 1.38; 95% CI, 1.29 to 1.43;=.01). The risk was greater in those with a baseline body mass index (BMI) of 28 kg/m2 (HR, 1.52; 95% CI, 1.43 to 1.69;=.02) and those with a BMI of 28 kg/m2 or higher (HR, 1.43; 95% CI, 1.28 to 1.61;=.02) but not in those with a BMI of less than 28 kg/m2 or a BMI of less than 27 kg/m2. In addition, the risk was higher in those with a baseline body mass index of 28 kg/m2 or higher (HR, 1.44; 95% CI, 1.25 to 1.60;=.001) and those with a BMI of less than 28 kg/m2 or a BMI of less than 27 kg/m2 (HR, 1.42; 95% CI, 1.27 to 1.57;Conclusion: A statin was associated with a higher risk of death or non-adherence in patients with cardiovascular disease and who are starting on statins. The increased risk may be attributed to the increased dose of statins used, the longer duration of statin therapy, and the increased risk of cardiovascular events.
CASE REPORT:A 52-year-old woman with hypertension (high blood pressure) and elevated cholesterol (HDL) was admitted to the cardiac unit of the cardiology unit of the University Hospital of Luanda. The patient was on hemodialysis for the past three months. On admission, the patient was found to be stable with no sign of coronary artery disease, severe heart failure, cardiac arrhythmias, or unstable angina. Her blood pressure was 110/70 mmHg and her heart rate was 18/min. Her labs were: blood urea nitrogen, blood cholesterol, antidiuretic hormone, and glucose. Her urine analysis showed moderate to high levels of sodium in the urine of the patient. Her urine glucose was 2.1 mmol/L. Her blood pressure was 130/60 mmHg and her heart rate was 15/min. She was admitted to the cardiology unit of the University Hospital of Luanda. A laboratory test was positive for antidiuretic hormone. The patient was admitted to the Cardiology Department of the University Hospital of Luanda, where the patient was given insulin for insulin resistance. On discharge, she was diagnosed with moderate to high levels of sodium in the urine of the patient. On admission, she was found to be on hemodialysis for the past three months.
